Active page just refreshes instead of moving forward

    The following page is not functioning as it should and I am not php knowledgable unfortunately.

    http://www.websuitcase.com/join.php

    When you check 'I agree' and hit submit the page just refreshes, can anyone see what I have wrong?

    Thanks for any help.
