A simple PHP script for your HTML feedback forms.. FULL OF FEATURES!

    I have designed a PHP script used to handle ANY HTML form on your entire website. You can use this with any type of form field (textbox, text area, radio buttons, checkboxes, etc).

    You DO NOT need to know much programming to use this script!

    This one script has the ability of handling ALL of the forms on your entire website!

    Here are the main features:

    1. Use with any HTML form using any form fields
    2. You can specify required fields for your form, and a page for the visitor to be redirected to if they do not enter a required field.
    3. You can configure MULTIPLE email addresses to receive the form results.
    4. You can configure the success page for the visitor to be taken to after they complete the form.
    5. You can ban MULTIPLE IP addresses from using your form! Great feature if you are receiving spam!
    6. You can configure the script to be sure that the form that is calling the script resides on your domain, thus preventing anyone else from using your script!
    7. You can configure an autoreply email to be sent to the visitor who submitted the form. You can specify the "from" email address, email subject and email body that is sent to the user!

    The script comes with the PHP file as well as a sample form, sample success page and sample error page. The PHP file is heavily commented with instructions to customize all the settings to fit your needs.
