I am trying to create a paid online quiz. So, I am looking for someone to create a form on my site. The form should have all the expected security and other features. I do not mind if you use an existing script such as the one found at: http://www.dagondesign.com/articles/secure-php-form-mailer-script/ But, I will need the inputted data to be stored in a database. Also, I will need some access protection features, such as passwords and image verification. Also, I will need to store the computer name/number from where the user first logs in. On subsequent logins, a user should only be accepted if logging in from the same computer as the first time around. Also, most of my questions in the form will be of the radio button format. In the quiz question as well as in its answers, there will only be images and no text. Naturally, there are other specs, but I think I have listed most of the major ones. Let me know how you would make this, language, price, etc. Previous work would be appreciated. Also, making a good looking form will be desirable.
