    @larasati: Thanks for being the first to submit your design. However, the layout and color scheme is not quite what I am looking for. If you pattern your design more after lvat.org. You will see what I talking about.

    @larasat: I do like it but I would prefer if it went all the way across the design. That way the user isn't distracted in filling out the form. Plus, the Hindu god statue is bit different.

    @larasati: The Hindu elephant statue really doesn't fit Life Insurance, sorry.

    @Jexxy: Thanks for sending this over. Your designer did a good job, and the client was able to see the full version. No revisions needed.

    @willhayes: This is good, I particularly like how you created the form. No changes needed.
