$50 to the person who can do this for me

    I need a form for my website, VERY badly. The person who can get the code to me that fulfills my requests will win $50, and if it is done very well, $100 is not out of the question.if there are multiple people in the running, I will pick the best one. Here is what I want with the form:

    - I want 3 text boxes with the names "Your name:", "your email address:", and "your message".
    - I want a field asking them what color font they prefer, and they have the ability to choose the color from a palette.
    - then I want them to be able to upload or attach up to 3 images to the form
    - lastly, I want the submissions to be sent to my email address.

    I DO NOT need any kind of payment integration, just what is above. Can someone do this for me?

    The "What color font they prefer" changes the color font in the message it sends you, right?

    Yes it is..forms have been submitted, but they do not work. Please pm me your form and we will see if it works. I don't know how to use my FTP, so if you can just make a php or HTML code that I dan just copy and paste, that woul be the best and quickest option.
