Pretty basic stuff, but it might help some people here.

Be careful when using #3 though. This article was written over a year ago, so back then it was okay to use images next to ads. Now it's not.

I think the example given in 1.ii is also now against the TOS

Also the part about smartpricing is not correct - smartpricing is concerned with conversion rate for advertisers, and adsense have definitely stated that it is not to do with the CTR on your site - you can have a low CTR but if every click converts the advertisers will be happy and you won't be smartpriced.

In fact I think it is more likely the opposite - if you have a very high CTR on a site, there is a higher chance that site visitors are being deceived/confused into clicking, and these will convert poorly for advertisers.

Whatever, you can't easily conclude that smartpricing is due to low CTR on a site.

For #3: We can add images if we break it with a line.
I mean a line between ads and images.
