OK I have a simple good looking header that I need made for this site:

I am doing a "freshening" to the design before we do the full re-design of the site. You can get the new logo here:

Please include a picture the address and number in the header. Something like 900px wide would be good. Make sure to match the colors of the site and you can get pics form their old site as well.

I need this fast so I should end the contest within two days.
