I've got a client wants an insurance affiliate form designed. Must have: three choices: Car Health Home 5 character field for zip code a submit button that says either quote or submit It may also have a title of 'Insurance Quotes', but that's not vital. I'mb being vague as I'm being open to suggestions. Perhaps a nice clean css/html coded form. Perhaps something that looks like a button since there's really only two questions. I'm willing to consider all entries. Personally I like some of the stuff on wufoo and from the forms design book at http://rosenfeldmedia.com/books/webforms/ . Contest closes Friday probably. We may not neccessarily choose a winner. However - the client has a number of sites, so we may actually be interested in multiple designs (and will pay each one $40 individually)- so don't hesitate to put in multiple entries. You agree that full copyright gets transferred to us upon payment. And check my itrader, I pay, and pay promptly.
It may also help (not a dealbreaker) if the form fits inside 300X250. The contest isn't for that specifically, but the client may have use for the form on a site where they have a spot of that size open (in addition to where they plan on using it - which is why this should only be a secondary consideration).

I know it's difficult, but I'd rather not specify the color. the form may be reused on other sites. If we go with your form, may have some color changes done to it if necessary.
