$35 Squeeze Page Design Logo Wanted

Hey,

I need a squeeze page made for a new sports picking service I am running. I want the users to sign up to receive free picks and want the page to look something like this:

Except my site needs to be horse racing oriented instead not basketball as the example shows.

I may award multiple winners depending on the level of entries.

Everything on the page needs to be easily customisable including the text that is written on any images.

This competition will close when I have found what I am looking for or after a week has passed whichever one comes first

 

So do you want just an image & .psd, or coding done as well?

Yes this contest finished a while ago, all winners were notified via pm
