301 Direct and Google Problem

Many, many months ago I changed the URLs on several hundred pages of one of my sites. I deleted the originals and used 301 redirect to direct Google to the new pages. However, many months later I have found that Google never took the new URLs into its index and, in fact, the old URLs are still in the index, but they are listed as supplemental results. When you click on these old URLs that are listed as supplemental results, you get a 404 not found instead of the new URL that they are redirected to.

Is this a glitch in Google or something I have done wrong?

Shouldn't get a 404 if the 301 is still in place. To be honest I can't remember the last time that I removed a 301. I realize that I should probably check the traffic on the old links but once I put a 301 in place I pretty much forget about it.

Without shouting out the obvious, I would just create the 301's and let them be. I did check a couple that have been up for 2-3 months. About 50% of them have the new page indexed with the old as secondary and the other 50% still show the old page but when clicked you go to the new page. I would guess that it would happen quicker with more traffic as well.
