$20 Email/RSS signup box design

    Closing: Sat 14 Feb 1pm EST. Prize: $20 by Paypal plus a linkback from my site (future post on blog transition to thesis once complete).

    Time to test your creativity!
    I'm looking for a graphic box to incorporate email signups.
    It has to mention that there's a free report "7 stages of Self Esteem" for subscribing (if you include a graphic cover and are selected, there's an extra $10 to send me that image separately too).
    The form will have aweber code embedded in it, similar to (but not copied from)

    My preference is for nice clean lines, classic look. Silver or deep red colour scheme would be great. It's to fit into the centre cell at the top of my blog at http://crystalsquest.com/sandbox/ (height can expand, width must be max 300px.

    Not quite. I want an illustration to put around my email/rss signup form. There are examples on the sites I listed. It needs room for the fields, but I want the graphic so:
    a) the signup box gets noticed
    b) it's clear that this form is where people sign up for the newsletter
    c) convey the offer that there's a free report for signing up.
