Buying 11 simple HTML forms needed

    In the Excel file there http://client.e-stonia.co.uk/apoint/gb.xlsx are 13 forms. First form is here http://client.e-stonia.co.uk/apoint and in Excel file can be seen all 13. Now you need to create from 3rd form 11 of them. First and second are done.

    The 11 must be exactly like the 1st one so that if in excel file the text field "naam" then:

    <p><label>Naam: </label><input type="text" name="naam" id="naam" eform="Naam::0"></p>
