Contest $100 -(24 hours) Need Custom Form

    Alright, so this contest is going to be a little more work than the average logo, so it’s $100.

    I have a form already made with a member who won a contest for the logo. So what I need is a custom template with the same information that’s on the one I will attach. It needs to have the same content and logo, but a different layout. I’d like to see free styles of the layout. You can change anything and everything, but it needs to look professional and nice, but also be custom.
