Buying $10 to the person who can fix my form

I am currently having a guy speed up my website and he did something that really
messed up my form at www.TheLoveQuilt.com/get-a-box.Instead of the three "upload image" fields there where my customers upload their images from their computer, there's an image of my websites homepage there..? I don't know why it wrong, but I will give $10 to the person who can get it back to normal.

 

Sounds like he has accidently uploaded a cached version of your page as a gif image. Weird

 

So can you fix it?
