$10 to a coder who can fix a non-working form

    Its a Javascript order form that's:

    1. not showing up correctly but working in IE and
    2. not showing up correctly and not working in Firefox.

    I guess you'll have to budle the JS into an 'include' file and call it as right now its all over the html.
