Hello, I am looking to get a Opt In Form Designed in Photoshop (PSD), that I can take to my web person and have it coded and put into wordpress. Now, the Opt In Graphic is going to need to be a total of 600px wide, as it is going to go at the bottom of our Service Detail pages and you can see where it is goign to go and how it is going to fit, and colors and everything by going to the following link: http://www.sismediagroup.com/our-services/search-engine-optimization/ It is going to go on the left side at the bottom under the content. Now, we are going to need it to have a Headline of the following: "Get your FREE Website Analysis Report Today!" Then we are going to need it to have a spot for First Name, Last Name, and Email Address. And it needs to have a photos of some sort of report or analysis or something.
