$10 - Need Two PHP Contact Forms for Website

I have a site that has two very simple contact forms. I need someone who can program the php contact forms with the existing design. I just want to contact forms to send contents to an email. I have a free script that you can use or you or you can use other code. I just need it implemented onto the website. I would like for it to have some sort of security feature like captcha to prevent spam.

I need someone who can do this as soon as possible. I'll give the details in PM. Payment made after job is completed through Paypal only.
