Buying $10 Job -- Need to add cpanel like file upload page

    Hi Friend,

    I need a file upload page same like cpanel, where I can upload multiple files, like starting with three files upload fields as cpanel and then a link, when clicked it will add another box, so that user can upload additional files along with the file title text box.

    with an html page for form, then php page to take care the file upload functionality.
    along with mysql table:

    uid, fileid, filetitle, filename
