I let somebody else to answer this professionally. I believe I cannot answer this up to your satisfaction. You cannot have this by your own. Google gives some sites an authority status when they've great quality backlinks from some of authority websites and relevant sites. It is acquired after pretty hard work and fair seo and website promotion strategies. In simple words, it takes time to achieve that authority status. Cheers!
Well Chilling breeze has answered your second question; you have build that authority before Google starts showing that for your website. Even after you have done that it would not do it for all organic results. I have only seen it happen when someone types the exact name of the company. So if you do a search for Kneoteric you will get those links but not for any other keyword search. Secondly the url structure. I don't see the point in having the index.htm at the end. The url without it just looks fine for both search engines and users. While checking out the links I noticed that I could access two URLs; one with index.htm and one without it. You don't wanna do that because it is duplicate content.

And I started to think that maybe this technique might cause high rankings because all of them are "index.htm" in different folders. Maybe Google gives the same value to all of them as Google gives to the main index page. This is just my prediction, putting forward my ideas. I think you mean sitelinks, not sitemap, which is a completely different thing. Google gives sitelinks in it's own time, they've never said exactly what triggers it, but clear clean structure and good navigation seem to be factors and there also seems to be an age factor that kicks in. It's got bog all to do with 'authority'

Sitelinks and sitemaps are 2 different animals. Many sites have them when doing a search under the exact domain name, or something really similar. The links google chooses usually appear on the homepage with good navigation. I did not like the ones google had for one of my sites and I made some changes to the home page links and now I like themMade the changes a little over a month ago, and this post made me look at them and they are better now. I have no idea when they changed them....

For First Question -> I don't think google and other search engine will give more imprtant to you subfolders index page.. Subfolders are use to organise the link structure of our site.. Search Engine algorithms are very strong and works different from any one's normal thinks.. They work on Artificial Intelligence Alogrithms.. If you need to set importance of different pages on your site, you can set priorites in the sitemap. This can help search engines to know the Important pages of the sites according to the webmaster and gives more attention to those pages than others..
