Photoshop / Illustrator for designing, then Codelobster for coding (VERY good piece of software considering it's free - tabbed interface, colouring syntax, predictive / autocomplete coding etc)

Been having a play with the Zend Studio recently actually, which looks good

I like coffee//cup html editor 2008. Its easy to use and code snippets that you can insert. Works a little like dreamweaver and a little like front page(if I remember correctly)

Adobe software for sure. The question should be, do you use Fireworks or Photoshop?

They both have advantages for me. Fireworks is better for slicing and controlling the layers in larger documents. Whilst Photoshop has many more visual effects for more detailed designs. I find I use the both for some web layouts.

Designs in Photoshop and then just pure code for me - Programmers Notepad. Just means total control and no excessive markup...
