Depends what type of plan you purchase, it its a montly plan they will more than likely charge you a set up free. If you pay yearly probably free set up. All host are different it just depends what your looking for, Try downtownhost there pretty good.

It really depends on the host. There is no set industry standard.
You should take some time to read the fine print.

For the most part, you either get charged annually or monthly.
Set up, (if you mean configuring your site/domain/bog etc) typically it is all on you.
Some will offer set up. Some hosts also offer to handle transfer of your site to them free of charge.

I like Bluehost - because it's $6.95 a month (I think) unlimited domains, simple scripts integrated. But I like the customer support. They aren't perfect, but for the price, it's pretty good.

Usually setup fee is according to host. You can pay via quarterly or monthly but hosts usually encourages payment in long term and even provides discount as it helps reduce duplicate cost@ cost to handle an unknown account.

With most hosts if you pay for a year or more upfront there is no setup fee.

Most web host offers monthly fees and some do offer annual fees as an option and usually this comes with a slight discount. Nowadays most virtual hosting plans no longer come with setup fees because of the level of competition the industry is getting. Setup fees would usually accompany dedicated server plans as those are more costly to setup.

Month to month is typical scheme of billing. But is annually you have 1 or 2 months for free. But at the same time that is very risky because you might be not interested in 3-4 months and won't be able return your funds.
