Currently I am using Adbrite and Freedom of Ads, as they both accepted me fairly quickly. With proper configuration of my account, I found they pay roughly the same for banners, however adbrite pays well for full page ads.

Other than adbrite and freedom of ads, you can go for canep media, media shakers, global interactive and best of all (If you can get in) try the pop unders from adversal. I haven't applied yet but I heard they pay best of all.

As for the physical code placement, google it, shouldn't be too hard to find out. I use glype so I can't help on this

Adbrite are on mine. It doesn't earn much though. Google ads on the frontpage earn the most for me.

I reccommend Adversal on proxified pages, if the accept new subscriptions.

The code can be inserted on index.php just upon this line
