$15 to make a document printable

I had a document created, but when i try and print it it comes out blown up and screwed up. I will give $15 to the person that can make it printable on 8.5x11 paper, the size of the document. thanks in advance! www.grizzlycomm.com/invoice.tif

People have come close, just need the text ledgible, and the graphics to a great quality. Also, need the file in .tif or/and .psd. thanks!
