Contact Form

  1. #1
    I want to add a simple contact form to my website.

    It must be as follows:

    Username/Name - Where they add there name.
    Email Address - There email address.
    Subject - The subject of the message.
    Type "Hello" here - Hello (goes here, spam prevention).
    Message - Message goes here.
    Send - Click to send to my email address.

    Can anyone help please?

    My website is coded in CSS, and xHTML. Using PHP for includes etc.

    try this http://www.emailmeform.com/ You can customize it and route the messages where ever you want
    It´s free. And it has captcha protection too !

    I am using their forms on all my websites without any problem.

    100% recommended.

    Uh, of course. But if you want it 'free' I can only provide you the mailer form as in a plain one, no massive design whatsoever.
