    this is normal, you have to porvide a phone number and tax information, then request a pin number from them and then enter the pin number when you receive it.

    paypal do a similar thing to verify you are not a fraud

    Go to Google Adsense for your account, click Contact Us on the bottom, and tell them that you had nothing to do with this, or you will surely be banned. If you let them know you noticed some strange activity, they'll be o.k. with it, although they might take your money away. Better your $2.63 than your account.

    definitely it's click bomb. but since you already informed google it's ok. I'm sure they are investigating the clicks origin. if it's click frauds, they will just reduce it from your total clicks and you wont get paid for it.

    this one just mean you set your google account payment tobe onhold. instead of giving you a check once 100$ have been reached. you can change this setting in your google payment details. nothing serious and no it's not a ban.
