Simple html designing change and also need to make some forms live on my site

    I have a website. For that I've the template in my hand. I need some very basic designing change on that one and also there are some sorts of forms there (Contact us, Request a Proposal). The forms are already designed on the template. I just need to make them alive. I guess there should be some scripts. I need the queries sent to my mailbox, not to any Database.
    I need this job done very urgently.

    Please quote your price asap.
