I think NO FOLLOW means when you're indoors, Don't friggin follow me around.
EXTERNAL NO FOLLOW means, if I go outside, stay the hell away from me.

    Lol...interesting way to put it

no follow links are usually used to tell the search engines not to follow a link within your own website ( an internal link such as your contact us page, terms of service page, privacy policy page etc.) The links are still found by all of the search engines, however it is not included when it is time to calculate the Pagerank for the website.

An External no follow refers to a link that is not located within your site. It is usually used for outer links from other websites. It basically tells the search engines that this is an outer link to another website ,also known as an external link, and you do not want to leak your page rank to this site so don’t consider it and don’t share PageRank with this link.

I am no expert on the subject so this is just my opinion about it.

Skee's kinda close - the external value of the rel="" attribute was originally used by SitePoint as a hook for a JavaScript application that would use it as an anchor to open a link in a new window while still allowing the web page to validate against a Strict DOCTYPE.

The nofollow attribute value on the other hand tells the search enignes not to follow the link. It doesn't matter if it's a link to an internal page (on the same site) or an external page/site.
