I'm quite sure I need to make on-page improvements but I'm not sure what... also, if I change the title/ description what happens to all the titles/descriptions I already have in links placed on other sites? One of the issues is your site uses one sitewide header file it seems. To some search engines this indicates duplicate content possibilities which can hurt you in doing well in the SERPs. Your home page title tag is using too many words and characters which dilutes the weighting & density assigned to the tag.
