I would like to quote some points that lead to the fail of SEO.This are the things I found in my SEO life.

    1) Simply Link Building without proper keyword research and On Page Optimization.

    2) Building huge links in less time (Link Spamming).

    3) Hiring people who don't know anything about SEO.

What all things you thing lead to the fail of SEO ?

I usually make my clients aware of this, before ordering anything from me.

generating quality backllinks, proper on-page optimization i.e, using appropriate meta tags, header tags, using quality site related content, design structure, navigation etc are also the key factors in seo which will help websites to reach on top and following off-page optimization which is link building is also useful. Using proper link building strategies is very important.

Other causes could be links from sites with links to dubious sites, identical link and accompanying text for all links, participation in Link farms.

Some time people use some black hat techniques to boost ranking.
Increase in keyword density more than 6-8 % will consider as a keyword stuffing will lose the ranking.

Keyword research is absolutely essential! Some time ago I had a web page that had a keyword that tickled my personal fancy. But then using a keyword tool, I discovered that no one was searching for that keyword. So I used that same keyword tool to find a good synonym. I found one, and noticed that it gets almost 8,000 daily searches on the internet. So I switched to that keyword, and since then that web page has brought me a flood of traffic ... just by changing from a useless keyword to a dynamite one.

Keyword Research is really the most important. Not only knowing how much traffic you will get from a keyword but also what type of audience will that keyword give you - will the traffic that you get from that keyword converts
(assuming your selling something on your site or want your traffic to take action)? The SEO efforts are wasted if you don't get any traffic from that keyword or even if you do they don't convert.

SEO is an ongoing process that need regular work and implementing changes and tweaking, what I see people do SEO and leave it, this is the main reason even good SEO fails!

Exactly, you have to cope with the new changes and try to adapt or you'll see your rankings drop in a matter of time.

Good SEO should always lead to conversions for the Client, therefore bad spammy content, built just for search engines is possible one of the worst techniques. (Good Visibility - Low Conversions).

Linking to unrelated sites and spammy content could be one of the reason.
