Contact script with image verification?

    Is there any very simple contact script I can use which will allow a form to be added to my website so people can contact me? I'm looking for something where I can just insert the fields into my existing template and then when a user submits the information is emailed directly to me (in a form where the users email is put as the 'sender' or 'reply to' so all I have to do is hit reply to get back in touch with them). I'm not looking for anything fancy, just a name, subject,email, message and image verification set of fields.

    Can anybody recomend anything?
