    I'm keen to get more feedback about what visitors think about my website. Can anyone suggest a script for serving a pop-up window once a user leaves the website which would include a simple form to send through a couple of simple questions about their experience.

    Maybe someone has a better idea about how to achieve this because people just don't visit my feedback page lol

    This is not the answer you're looking for, but I would suggest sparing yourself the effort. I can't imagine anyone bothering to complete a site exit poll - I certainly would not.

    I used an exit poll with great success.

    You need to use javascript to create a popup window and call the script when the window is closed using something like onclose();

    I only included the script on the confirmation page of the site (ie once a transaction was completed) and I used cookies to make sure the popup was not shown twice to the same user.

    Me neither a popup is still a popup even paved of good intentions. Also there is 99% chance it may be blocked by popup blockers.

    I totally agree. Exit polls are (1) irritating, (2) amateurish looking, (3) likely to be blocked anyway, and (4) unnecessary. If people come back, they liked the site. If they don't, they didn't.
