You would want to do a 301 redirect for all of the pages that are to be changed. If you are using a CMS, such as Wordpress, there are plugins that may help you. I know Yoast Professional for Wordpress will automatically redirect as you change it. And yes, if you don't do the redirect it will affect your SEO.

Yes, it will affect big time if you are not doing any 301 redirections from the old to the new permalink. Given that you have properly applied 301 redirections, there will still be a little dancing from the ranking and traffic of your new site. However, if the content, the meta are in tact, it should be back on track.
One lazy answer, if there aren't too many posts, is to put up pages at the old-format URLs and put in the 301 redirect code: <?php header("HTTP/1.1 301 Moved Permanently"); header("Location: http://www.New-Website.com"); ?> I'd be lying if I said I've never taken that lazy route. :-D Might be faster than finding just the right wordpress plugin.
