    Depends on your theme and content placement. The best ones however are skyscraper and ads in between posts. Make sure to customize your ads to the theme of your site to increase chances of visitors clicking on the ads.

    250x250 , they aren't to big and not too small, just fine . works very well for me.

    It's obvious that banners size have a huge value but the more important is where exactly on a site a banner will be placed.
    And even more important the design of the banner. So there won't be one answer.
