Hey, i need a submission form made where members can upload their photos and other details while filling up the form. Once the form is filled, i should receive it on my email.

What's the best way of achieving it?

Custom script or pre made add one's from wordpress.

Please link me your suggestions as well.

    Thanks.

What CMS are you using, or is your site standard HTML?

I haven't decided it yet, the only purpose of this website is setting up the form, nothing else.

In that case, I'd definitely go with Clip Bucket (http://clip-bucket.com/).

Like wordpress, there are a lot of modifications and plugins to do almost anything you might want, but unlike wordpress, most are not free. http://forums.clip-bucket.com/forumdisplay.php?58-ClipBucket-Plugins-amp-Modifications

I would have suggested Wordpress because of the vast amounts of plugins available, but I tried to allow pic uploads on my own site and couldn't find a plugin that made it possible. The only plugin I found that claimed to do so didn't work for all users.

You can do this by easily with php language, in this you shuld have a database connection a directory where you can save your images, first of all create a form in html language. then start coding in php and mysql you can get ideas on google searching form submission script in php. www.urdutops.com
