Buying Need Custom (or exising) Wordpress Form...

Hi Guys,
We have been building a website for a pet food company.
One thing they would like to have on the site is the following scenario;

On a page, there are two images (dog and cat),
When user clicks on dog (for example), it will load a list of diet types (there are 8 diet types),
When user clicks on diet type, will list reasons for wanting that diet (there are a few reasons for each diet),
When user clicks reason, it will provide a response (that includes their choices) and information about the diet.

EG> Because you were looking for a diet to help with *cancer* in your *dog*, we recomend using *this diet* (this is very dummied down, but you know what I mean...)

Perhaps some sort of slider or something already exists to accomplish this. That's ok, but it does need to be mobile friendly.

We would love this to be as graphical, fresh and EDITABLE as possible.
