    Directory submission is one of the best form of SEO. I tried so many other ways but to tell you the truth I have never succeeded without directory submission.

    No i don't think that directory submitting have dead and will be ever because directories are the great source to got back links and it is very easy way to get back link as well .
    After recent Google update this technique have become more popular and useful .And you should try to submit your site in the high PR and popular directories to got good results .

    No banned but Google banned and penalized around 2000 directories. Which is not sufficient and effective for website due its quality and reputation. I would like to recommend you to use those websites which have good reputation and domain authority.

    Directory submission is much alive. BUT, you must use websites which have good reputation and domain authority, like say other guys.

    No, its not dead. Its totally depend on you that from which website you are getting link( I means its PR, alexa and type of links it accept). Please beware from websites those accept adult and poker websites links and those have lots of ads on thier sites.
