    I'm trying to get a better idea of what people want on their contact forms. Please post here what you have on yours, and feel free to post a link to your contact form if you desire.

    My contact form is simple:
    - Name
    - Email
    - Message

    That is really all you need to get the ball rolling. Once someone gets in contact with you then you can exchange more information if necessary.

    That's all mine usually have but I feel like others would prefer a great deal more options than that. I am adding more options to my contact form generator soon so I would like to know people's preferences.

    Good question.Here are certain important things that an ideal Contact form should have:

    - Reason/Motive for Contacting (Adding a drop down of your services is a good option to make things specific)

    - Name of the person
    - Company Name
    - Country
    - Phone
    - E-Mail
    - Message

    Leaving a ‘Thank You’ message after the visitor submits the form or acknowledging the effort that the visitor made is a very good move on behalf of the firm.

    Oh, you should definitely add phone, subject, website, and the possibility to create custom variables as well. A follow up email would be great too :)

    I guess this depends on what kind of site you have: a tailor shop, or a wedding dress site might want to add upload box in their contact form,
    allowing their visitors to easily send their own custom/ design scratch. Also, know your audience first, before they see your contact form as a lengthy questionnaire:)

    Here's my 2 cents to this post: Does your website offer a service, product, or just information? What do you use the contact form? Is it a true contact form or are you using it to build your list?

    If it is a true contact form, just get the information you need to answer their queries. Some customers do not want to share all so they will opt not to fill your form and go elsewhere. If you only need their name and email to answer a question, thats all you should ask for.

    If it is a list you are building, put up a free offer to entice them to fill out the form. Free offers such as an ebook (can be a PLR ebook or one that you have written) on a subject related to your website that will give the customers an incentive to fill out your form. I offer a different .pdf every month on my list and people graduate from one series of emails to the next until I can put them in as a customer. For example person A has signed up 3 times for 3 different .pdf's he would be assigned to campaign 3, mail 1 and so on and so forth.

    There are a million answers to your questions but the right one will come from you split testing. Test a form for a week or a month, and if that isn't working, try another one until you find one that works for you and your customers.

    Every website has its own contact form. Some have it from Zoho and other who has a huge daily contact flow rest can have it from Aweber and rest from free tools.

    For me the main agenda of contact form is to get the information of our potential customer or the visitor. So grab as much information as you can such as email, messenger id or whatever so that you can follow up or contact them.

    Name email phone and message. The form should be made to look good. If you are using wordpress try contact form 7 or gravity forms.

    this is more than enough.. Too many fields may increase your bounce rate on contact page.
