    Yes image backlinks count as backlinks and here anchor text is nothing.

    Suitable File name for the image
    Perfect Alt tags for the image
    Title tag for the image.

    Try to add keyword in image as alternative text and also use it in url it is helpful for getting backlink.

    by optimize your image you can get more presence on SERP. Because Bot read ALT tags & image name. but your second question do not understand. you asking getting backlinks or some thing else ?

    Image optimization using ALT Tag is the HTML tag that informs web based search engine exactly what the images are only for.     Normally SE's don't look for your image . They just go for alt tags so your alt tags should be relevant to your page. It will be more effective in case of using keywords in alt tag.
