need javascript: multiple select with checkboxes.

I've select:

I want it to look like this:



I want you to write very light and not complicated code to achieve this effect. There are many examples on the net (I will provide in case your google skills aren't up to notch). I've found very nice one with jquery, but it's hard to customize - as I'm not very efficient with this framework.

2 questions:
How long will it take?
And how much?
