I sell web templates to local businesses and need someone to code a form (like a contact form where all of the info is sent to my email address) for my clients to fill out. I want the form to have validation -- if a field is empty it will give you a warning, or if the email is poorly formatted. The hardest part of the job is the part of the form where the client chooses a template (which will each be represented by a small clickable thumbnail). Each template will have a radio button above it.
Willing to raise the price to $10. It's a very minor job. You don't even have to code from scratch. You can use existing scripts if you want.
