    Yes! All of my top 5 CPA networks have many of these great easy signup offers. Maxbounty, Hydra, Azoogle, CPAE etc...

    All the major networks have them (Clickbooth for example have >100 I think). But you must choose the campaign regarding to your traffic. If the offer is related to what your visitors want/need it will convert nicely.

    many networks have zip and email submits. I am sure most of the networks have several. Of course some networks dont have them, but they are the exception.

    I'm averaging about $1.45/lead with the network I'm using atm, do these networks have higher payouts?

    some of them have high payouts but not much more than 1.45 some have payout of 1.50 or 1.60 for some of the offers.

    $1.45/lead is pretty good for a zip/email submit form (to get up to $3-$4 the campaigns ask for more data in the registration like name and adress or answering a survey)
