I need a contact form programmed

    Hi There,

    I would like a simple ajax contact form programmed. It must be well coded and be coded from scratch, with a validation query for the name, email and subject forms to be filled.

    It must have a name, email, subject and message boxes. It must look clean, sleek and professional. Once the first form has been created I want the form implemented in to creative contact forms, such as on a sticky note, boarding pass and a couple of other contact form designs I need doing.

    Payment will be via PayPal. Please let me know me costs and turn around. I have a budget set in mind of around $50.
