    The deign looks very nice, simple and attractive. You can change the body font color from black to white as it would looks mush better rather.

    That's very very nice! Keep up the good work! I made a online form creator as well that is able to create survey forms. If you could create more form control options that would be better.

    That's a strong head-start! I especially like the results screen.
