I have a template that I have been using to design a series of websites. I need to make a few small changes to the template. The template is in all HTML, but the background images and theme were created in photoshop. I need someone who is photoshop proficient to make a few changes to the theme, including, adding a CONTACT FORM at the top. 5 field submit. Then all you need to do is save it all back into the template and if it all works well then we have a deal.
