    Yes, it will affect or hurt your blog, because the links that you have built so far will be different if you change the directory.

    you can still use permalinks so its like site.com/first-blog-post yet the main index is /blog

    yes but i used .htaccess so what ever link come to site (post) it will move to same post under blog

    It may not effect. If you use some other keyword other than 'blog' will have a good effect too.
