According to SEO expert Bruce Clay, the words in your title, description and keyword tags and the content on your page should have a very close relationship. Then your H1 tag should be consistent with the just mentioned. It lets search engines know that you said what you meant and meant what you said. You should include unique meta tags on each page. But do so according to what they page is about. Meta tags still provide information to engines even if they don't use it as a specific part of the ranking algorithm. People sometimes forget that part of SEO is making sure that the SE's know what your site is about.

For description meta, you need to write different descriptions for different pages. It should be done handy, because your website has small amount of pages, and do it by hand, you can pick best description.

For keywords meta, you can use the same for all pages. But it's better if you can find different keywords for different pages. For me, it's optional.
