Wordpress contact form WITH redirect

I am looking for a plugin, but have reached a dead end. I need your assistance!

I need a SIMPLE contact form (name, email, company.) that will redirect to a page that has a DOWNLOAD LINK.

(this is to track the names of everybody who downloads the file. I know this can be done several ways, but I want to use a contact form)

Pretty much, all i need is a Wordpress Contact Form that allows redirects.
