Anyone recommend a good php survey script / application?

Ideally looking for free / open source but paid is ok too. Looking for a robust survey script software that allows multiple surveys and has a survey builder.

If there are any 'hosted' solutions out there that can be white-labeled (none of the providers branding) then also interested to hear about those.

Any helps or pointers would be great.

Please note, I am aware of script directories, hotscripts, etc etc but would like some recommendations from people to help educate a choice

For anyone that is interested I tried lime survey but found it complicated.

I have now settled on http://www.askpeople.co.uk and it is simple, powerful and perfect.

 

from my experience, I Give Test is the bst one.

my personal openion is limesurvey :- might look bit complicated at first but if you give it some time it will become easy.. and it is the best if you wish to add more then one survey
