Need help with PHP Order Form

    Hi DP,

    I need help from someone who has experience with PHP order forms. Here is what I need:

    1 - I need the order form to work with PayPal.

    2 - The order form will need to do certain calculations and then pass the total over to the PayPal form for checkout.

    3 - I'm using a form building software to create the form. I need someone to make adjustments to the form I've created.

    4 - The form software stores the values in a mySQL database, so I need the database modified as necessary.

    5 - I need the form added to my existing website.

    6 - Additionally, I want a collapsible div function that a user can use to hide and show the form on the website.

    I need quotes for this project. I need someone who can do the job within 72 hours. I need someone with positive itrader comments and preferably with a portfolio or work samples.
