    A report is not more than bunch of data displayed in sorted way. Just start writing everything that you know related to seo. Don't worry about editing, just write whatever comes to mind.

    After you are done go over them and group them and order them on which is the most important.

    Start analysing your site each and every link.Note all links with PR.further you also can do competitor analysis for your website keywords.

    site analysis Steps
    1. Understand site carefully.
    2. Collect seed keywords from main pages.
    3. Collect related kwds using Google adwords, Google suggest, Google insight..etc
    4. Filter keywords and try to pick up nearly 100 to 200 kwds.
    5. Check Completion, Traffic, R/S, KEI and keyword density values.
    6.And come to On-Page issues
    7. Check site design, Site structure, Navigation, Link structure, ATT tags, Ancher text and etc
    8. And search & collect Competitors(Business & Web)
    9. Research on competitors.

    There is no international standard or rule.

    I provide seo and website analysis and my report is different from what i have seen from other experts.

    Just do what you know...
